Social Services

HIV/AIDS produces challenges for people in the home and workplace. Our center offers social services provided by social workers and case managers who are experts in HIV/AIDS-related needs and services. Each patient is offered an initial social service assessment and follow-up as needed.

Other services include counseling, support groups, ADAP enrollment and assistance with Social Security and Medi-Cal enrollment.

Our case managers use contacts with community mental health, housing, disability, training and advocacy groups to provide access to the full array of services in San Francisco.

Peer advocates work closely with the case managers and social workers to provide daily out reach to patients and community based organizations. Peer advocates can assist with forms for transportation, housing, etc., remind them of their medical appointments, and accompany them if desired.

Support Groups

Sometimes talking with others who are dealing with similar issues can be the best medicine of all. 360 offers periodic support groups for those living with HIV/AIDS.
