Appointments

Regular Appointments

In order to insure your treatment plan is the best it can be, we suggest that all patients see their primary provider at least every three months.

360: The Positive Care Center at UCSF is open for appointments from 9:00 a.m. to noon, and 1:00 p.m. to 5:00 p.m. Monday through Friday.

Reminder Calls

We will remind you by telephone 48-72 hours before your appointments. In addition, please let the front desk or your provider know if you do not have access to a telephone or you have other requests. Please call and reschedule your visit if you can not make your original appointment.

Sick Visits and Urgent Problems

For urgent issues, please call the clinic and alert the staff. The nurse will review your needs and schedule an urgent visit. Your provider will try tobe available to you when you need to see him or her, but if no appointment is available when you need it we may recommend that you see another 360 provider or go to the Urgent Care Clinic or the Emergency Room.

Call 353-2119 during work hours.

Call 476-1000 at night, holidays and weekends and ask to speak with the 360 provider on call.

What is an Urgent Problem?
Not all problems are what we call urgent problems. However certain conditions do require that you receive health care promptly. You should call the clinic (during work hours) or the on-call physician (see below) if you experience any of the following problems

  • Trouble breathing (that is new or more severe than previously)
  • High fever (a temperature over 103°F or 39°C)
  • Severe headache (more severe than any previous headaches)
  • Fainting or loss of consciousness ·
  • New weakness or numbness in part of your body ·
  • Vomiting that prevents keeping your medicine down
  • Diarrhea of more than 3 stools per day for more than one day
  • Chest pain (more severe or long lasting than pain you have had before)
  • Blood in your urine

If you must get help immediately, call 911 and tell them that you are a patient at the UCSF 360 program (formerly known as the PositiveHealth Practice at UCSF).

What to Expect at a Visit

Please stop at the front desk when you arrive. This gives us the opportunity to register your visit, update your information, and send your chart to nursing for check in. Be sure to tell the front desk if you have moved or have a new telephone number or if you have an urgent problem.

Your provider or one of the nursing staff will call you to begin your visit. The nursing check-in is a good opportunity to discuss any problems you are having, including issues with your medicines.

Remember to stop at the front desk again, after you have completed the visit with your provider. Front desk staff will assist you in arranging other appointments and diagnostic tests. Please obtain the necessary phone number(s) at the front desk when you check out. In addition, take one of the 360 business cards with you to the appointment(s) and give it to the specialist who sees you. These cards are available at the front desk.
