
Medication Services
If you are currently receiving medication, your pharmacy can provide
refills or call us to get authorization for refills. Please call your pharmacy when you have an existing prescription to confirm that your medication is being prepared. Refills for restricted medications and initiating new prescriptions may
require a clinic visit.
If a written refill is needed, please notify the clinic in advance, so
the prescription can be prepared, and your provider has time to discuss
any changes with you if they are indicated. We prefer you notify us one
week prior to needing a written prescription, but we need a minimum of
48-72 hours. Arranging for refills and new prescriptions depends on both
the clinic (by responding in a timely manner) and you (by notifying us of your need).
- Individual assessment and counseling sessions with pharmacist
- Drop-in group sessions with pharmacist
- Treatment adherence program—review goals for taking medications, review barriers for taking medications, address barriers, side effect assessment and treatment, assessment of response to medication, lab and blood tests as needed,
- Pharmacist follow up with patient and primary care provider by phone and/or one on one
- Yearly visit with pharmacist
