Services

The Men of Color Program uses a comprehensive care model that is important for our patients because of its integration of social services, expert medical care, and specialty services in one location. Our philosophy is to use culturally sensitive approaches to support clients in defining and achieving their healthcare goals.

We hope to minimize the number of men of color who are lost to care.

Our Goal

MOCP strives to connect HIV positive men of color, in particular African American men who are not in optimal care, to primary care services, and to support them in developing and maintaining a long-term relationship with a primary care provider.

Services

A multidisciplinary clinic which includes the following services:

  • Expert adult HIV/AIDS primary care
  • Yearly psychosocial assessment and thorough medical evaluation that includes nutritional and pharmacy assessments for treatment and health advocacy
  • Counseling
  • Social Services
  • High-tech medical procedures
  • Support groups lead by our social services team for clients to discuss common issues in an informal and supportive setting
  • Collaboration with Shanti in the area of onsite health counseling and outreach.
  • Work with Quan Yin Healing Arts Center to provide onsite massage and acupuncture therapy.
  • Referrals to UCSF medical specialists
  • Close links with community agencies in San Francisco
  • Access to new and ongoing research studies and medications
  • An on-call physician, case manager, and peer advocate available after hours, seven days per week for urgent clinical matters
  • Periodic onsite health/wellness counseling and onsite massage/acupuncture through collaborations with some of our community partners

Primary Care Provider

Our primary care providers take care of your HIV and primary care needs with regular check-ups, while collaborating with other staff members to ensure your care. They will also offer information on medical studies.

Social Worker

Social workers help us understand you better, refer you to community services and meets with you for urgent social services and mental health needs.

Case Manager

The case manager assists with other services outside MOCP, such as job training, housing, substance abuse, benefits counseling, etc. They also can help with managing your "life" issues.

Peer Advocate/Outreach Worker

Peer advocates will assist you with forms for transportation, housing, etc., remind you of your medical appointments, and can go with you if desired.

Outreach workers guides your relationship with the MOCP staff and contacts other outside services you may need. They will talk with you about your problems with getting care and helps with those problems.

Dietitian/Nutritionist

Our nutritionist will help you choose healthy foods to eat, check your body fat and muscle, talk with you about weight loss or gain, eating or digestion problems. They can also help with conditions such as liver disease, diabetes, and high blood pressure.

Pharmacists

Our pharmacist talks with you about medicines, how they work, and possible side effects, answers questions about medicines, and helps you fit your medicines to your lifestyle.

Medical Assistant

Our Medical Assistant will handle most of your routine needs. Such as checking your blood pressure, weight and other basics when you visit. Helping you with medication refills and approvals, giving you injections, (flu shots, etc.), tests (TB, etc.), and also assit you in getting test results and information.

Practice Nurse

Urgent matters as well as routine needs may be met by our practice nurse. They will give shots, and help with medicine refills, approvals, treatments, etc. Talk with your doctor to be sure he/she is aware of your medical conditions or needs, and offer information on medical studies. And finally, instruct you on diabetic care, smoking cessation, and other related activities.

Front Desk

Our front desk helps to make appointments, referrals, scheduling tests, and refers you to the appropriate provider and staff.
