Partners with MOCP

Our working model with Community Based Organizations (CBOs) presents great opportunities for efficient cross-referrals of clients to ensure that easier access to high quality care is provided. As a partner agency we are engaged in an active network to provide HIV positive Men of Color in the Bay Area improved access to quality care and services.

Recently, 360/MOCP expanded our services by collaborating in the Bayview-Hunter’s Point (BVHP) Center of Excellence, the Southeast Partnership for Health (SEPH) for HIV-positive and affected men and women in the BVHP area. As a result, we have expanded our services to provide care for African Americans living in the BVHP area who prefer care outside of their neighborhood (e.g. for confidentiality) and access to the clinical and research expertise at UCSF Medical Center.

By participating in the Southeast Partnership for Health, we have been able to expand our services beyond the Western Addition, Tenderloin and Mission Street areas that have provided most of the MOCP clients and have strengthened collaborations with CBOs across the city. In doing so, we have augmented the resources to which we can provide access e.g. direct access to substance abuse treatment, more extensive case management, and increased peer advocacy services.

MOCP also continues its ongoing collaboration with Shanti in the area of onsite health counseling and outreach. We also work with Quan Yin Healing Arts Center to provide onsite massage and acupuncture therapy.
